About the Community Resource Coordination Program: The State of CT is undertaking widespread COVID-19 testing of symptomatic and asymptomatic residents of Connecticut. To prevent the further transmission of COVID-19, the State, through the Department of Public Health (DPH) is conducting contact tracing. Contract tracing includes, but is not limited to, contacting and engaging with COVID-19 Cases and COVID-19 Contacts. A COVID-19 Case is an individual that has tested positive for COVID-19 (“Case” or “Cases”) and COVID-19 Contacts are those individuals identified by the Case, who may have been exposed to COVID-19 through contact with the Case (“Contact” or “Contacts”). The role of the Contact Tracers is to encourage and emphasize the need of individuals identified as a Case to self-isolate and individuals identified as a Contact to self-quarantine in an effort to prevent the further transmission of the COVID-19. Contact tracers will also identify Cases and Contacts that, in order to successfully self-quarantine or self-isolate require services and supports. The Contact Tracers will refer those Cases and Contacts to the appropriate Community Resource Coordinator host organization as determined by the location of the Case or Contact, to receive resource coordination services provided by a Community Resource Coordinator. Gilead Community Services is the host organization for Region 2 which includes Middlesex and New Haven counties.
