Nursing Home Diversion Program
Working For Integration Support and Empowerment (WISE Program) is a nursing home diversion program for individuals living with mental health needs.
In-Home Supports
The Wise clients thrive with the various in-home supports provided enabling them to live safely in their own home. With these supports, clients learn to cook, organize, clean and manage their personal hygiene. Skills for shopping, appointment management, community integration, improving social supports and coping skills are developed and enhanced.
Transitional support is also offered to nursing home residents to help them move into their own homes.

Community Activities
While some clients may enjoy fishing others enjoy Ping Pong at the local senior center. Clients come together to prepare and enjoy a dinner and a movie once a week. Sometimes finding a movie that suits everyone’s taste can be difficult, but they ultimately enjoy the time spent together!

The Gateway Community Treatment Team provides a variety of services focused on rehabilitation services and recovery support to help individuals achieve and maintain independence in the community.
Our dedicated staff provides case management, community support, crisis intervention, group and individual counseling and skill building.
